Thank you for filling out a Level B Provisional application.

Once all signatures have been collected, the completed form will be submitted automatically to info@fulford.ca where a Fulford Certification staff member will review it and send a confirmation email (or follow up on any missing information) within 24 business hours.

Both signees will also receive an email with the completed form for their records, and a payment receipt will be emailed to person who initiated the application form.
