Level B Provisional Certification Application Instructions
Please read the following instructions before proceeding to the Level B Provisional Certification application.
The application is a Docusign form where you will need to fill out information about the crane, operator and employer. The application will need to be digitally signed by operator and a named representative from the operator’s employer.
Payment is required to submit your application. After filling out all required fields please click on the “Finish” button which will bring you to the payment page (see image below)
Please note, once we receive your application, you will be sent a confirmation email with further details and instructions within 24 business hours. If you do not receive a confirmation email, please review your application to ensure payment went through and both parties signed and submitted the form.
